Membership Funding Request Form
Funding is based on exenses that a Lodge needs help with attracting new members. Items include: promotional materials (brochures, giveaways, clothing, food for a social event, advertising, facility rental) *Alcohol will not be funded*
Once this form is submitted, the Member Services Committee will approve or not approve the requested amount. The amount agreed upon will be paid by the National Office after we receive receipts of expenses and the follow up form.
